2. The applicant shoud be a diplomate of Philippine Board of Urology
3. Accomplished Application form and pictures.
4. Certified true copy of certificate of membership in good standing in the PMA and its component society.
5. Certified true copy of the PBU Board Certificate accompanied by the original copy for verification purposes.
6. Must have a photocopy of his/her current PRC license.
7. Letter of recommendation from 2 fellows in Good Standing.
8. Three (3) 2x2 pictures (white background)
9. Payment of Application Fee P2,500.00
ASSOCIATE
1. A graduate of an accredited urology training program
2. Letter of intent addressed to the chairman of the membership and credentials and membership committee (Dr. Paul Anthony L. Sunga)
3. Duly accomplished application form
4. Certified True Copy of Medical School Diploma
5. Certified True Copy of Philippine Medical Board Certificate
6. Certificate True Copy of Residency training in Urology
7. Certified True Copy of certificate of Membership in good standing in the PMA and its component society
8. Must have a photocopy of his/her current PRC license
9. Letters of recommendation from 2 Fellows in Good Standing
10. Three (3) 2x2 pictures (white background)
11. Payment application fee (P4,500.00)
